Objectives
- At the end of the CPE activity the pharmacist (P) and pharmacy technicians (PT) will be able to: 1. Define the concept of transitions of care (TOC) and its significance in the evolving healthcare landscape, including the impact of suboptimal TOC on patient outcomes and healthcare costs.
- 2. Explain the pharmacist's unique position as the central nexus in managing care transitions, including their key roles and responsibilities in medication reconciliation, patient and caregiver empowerment, addressing access barriers, and coordination and communication.
- 3. Identify the common challenges in TOC, including poor communication and coordination, limited access to a patient's electronic health record, patient-related barriers, and inadequate access to post-discharge medications.
- Discuss the opportunities for improvement in TOC, including the expansion of pharmacy programs, and promoting collaboration and standardized processes.
- 5. Describe strategies to overcome barriers and improve TOC, including the use of medication reconciliation, patient education, and coordination and communication tools, to reduce hospital readmissions and improve patient outcomes.
