  • Explain the role of the pharmacy in optimizing the medication reconciliation process and supporting safe and effective transitions of care.
  • Identify challenges and opportunities in improving
  • transitions of care across the inpatient and outpatient healthcare settings.
  • Describe strategies to facilitate the access of medications when transitionindgs of care.
  • Explain how pharmacists and pharmacy technicians from different settings experience collaboration within pharmacy and other health professionals to facilitate smooth transitions of care.
  • Recognize the importance of collaboration and communication among all parties to facilitate TOC and access to medications.
